Office Closing
Dear Patient,
To begin, I would like to thank you for the trust you have given me over the years as your OB/GYN. Taking care of you and your family has been a huge source of joy for me. Your wellbeing and health have been priorities both for me, and the staff at Her Care Inc.
It is with mixed emotion that I wish to inform you that I will be closing my practice on October 31, 2008. I have been offered and have accepted a position at Washington Hospital Center in Washington, D.C., as the Medical Director of an OB/GYN Group Practice and Associate Professor at Georgetown University.
I understand that this might be unsettling for some of you, but rest assured, I have planned for plenty of time to facilitate the transfer of your medical records and am available to help aid you in selecting a new local physician. I will also be available, on a limited basis, to see patients by appointment until the end of the year.
We understand that medical information about you is personal and are committed to protecting that information. For your convenience, I have enclosed an authorization form for the transfer of your medical records to a physician of your choice. Retrieval of Medical records will be available through December 31, 2008 at the present office location. Afterwards requests will have to be made via e-mail at hercare@aol.com.
In closing I want to thank you again for putting your trust in me to take care of you. I will always value the information and emotion you have shared with me in confidence.
